ONE CLEAR FACE ONLY PHOTOGRAPH
- This photo is used on your Ambition Health Passport
- We can either use a passport photo, or we can take a picture of you at the interview, or you can send us a digital picture by email
Please make sure that you like the photograph, before we supply you with the Health Passport (Ambition ID Card)
SA ID DOCUMENT / SA DRIVERS LICENCE OR PASSPORT / WORK PERMIT (CERTIFIED) !!
If you don't have a SA ID Document or SA Drivers Licence please provide a certified copy of your valid passport and valid work permit.
Please note that we require sight of the original documents OR certified copies. There are TWO ways in which this can be handled:-
- The originals OR certified copies can be taken to any Ambition branch for copying and certification.
- Certified copies can be made by at any police station or Post Office, and these certified copies posted to us at:- Ambition24hours, PO Box 2194 , Bellville, 7535

PROFESSIONAL REFERENCE DETAILS - TWO !!
Ambition 24 Hours require references from your last or most recent employer.
By professional we mean actual employers, not colleagues, so work addresses are essential.
All references must be from the last two years.
If you have left a position working with children or vulnerable adults, reasons must be given.
CONFIRMATION OF HPCSA REGISTRATION !!
Please provide a copy of your annual registration certificate.
EVIDENCE OF PROFESSIONAL INDEMNITY COVER !!
We recommend that all Health Professionals that work for Ambition 24hours have Professional Indemnity (PI) cover.
This insurance, however, is not compulsory yet. The Minister of Health is in the process of promulgating an amendment to the Health Professions Act that will make PI insurance compulsory for ALL Independent Practitioners. This should become law sometime in the future.
If you have PI insurance please provide a copy of your certificate showing the expiry date of the cover.
If you decide not to take out PI insurance, please download the "Professional Indemnity Acknowledgement" here and sign for our records.

We require these details for our payroll department. Please provide full details including: Name of the account, type of account, account number, and branch code.
DRIVING LICENCE
Please provide a copy of your driver's licence.
CV / Résumé
Please provide a full and detailed CV. If possible please provide an unbroken work history covering the last 5 years. This will enable us to clear you quickly through the compliance process, and will be very valuable for marketing your skills to our clients.
